PPP tracks locally produced goods, like food, housing, etc. in the local currency.
So a poor nation may score relatively well after the PPP adjustments.
But what if you want to buy a Mercedes or Toyota car or truck, or a laptop with an Intel processor and NVIDA chips? Those are bought with hard money, not some local crap currency. (yes, I am hostile to the idea of pushing the useful PPP idea too far to be some universal criteria)
What if you want to buy world travel on airplanes, which requires Boeing or Airbus aircraft, and Jet Fuel - and jet fuel, which comes from oil, which is priced globally, not so much locally?
So if a person is middle class in a moderately developed nation, many of the things they want to buy are priced globally in hard currency, and PPP is irrelevant.

Course Duration
This program is five years long. Upon successful completion of the five years of training, a degree of Doctor of Medicine of the Tumaini University Makumira will be conferred. Upon completion of the five years the graduate is expected to undertake an additional sixth year of apprenticeship in an approved institution before being allowed to practice independently.
