In Kenya for you to do a degree in Medicine you need at least 82 points out of the possible 84 for you to qualified. When I did my form four I got 78 points out of 84 but I could not even get close to pharmacy.

Medicine is a very vast field of study. Your screenshot only talks about "minimum requirements" but you didn't say which course under medicine. You can't study a Bachelor of medicine and bachelor of surgery (MBCHB) with grade C+ in Kenya hata utembee hadi wapi, otherwise everyone would be a doctor in Kenya. You must score a mean grade of a straight A with straight As in sciences and there's no shortcut here
