What they mean is that for you to do medicine in their institution you past have passed basic medical knowledge from your previous study for example you must pass your certificate cause in Medicine for you to advance to diploma level. The same happens when you want to do the degree level, you must have pass the diploma level in order to advance to degree level. Do you know what that means? It means longer time studying.

I think you have problems with your eyes. From this screenshot Roman two us talking about advance while Roman three ndio inaongelea diploma and that's where your O' level is found.
