If he was in Tanzania, he was in a Tanzanian tour van with Tanzanian tour guides, I expect the Tanzanian tour guides have contacts in Dar hospitals and they're using air transportation so a difference in distance of 500km shouldn't be a big deal. The only reason one would choose Nairobi over Dar is either lack of efficient flying doctors in Dar or better hospitals in Nairobi.
