While ATC is legendary for its less than mediocre quality control and almost nonexistent safety standards - I can recall hearing a squeaking wing this past July to attest to this personally-, the aviation industry in general, even in the confines of the best maintained and well funded airlines, is not without genuine difficulties. A case in point is the recent snafu to do with Japan Airlines' 787 fuel and other technical issues the flag carrier is having.
It remains to be seen whether this incident is a characteristic of the former or the latter.
An analysis steeped in sobriety and empiricism would require a transparent technical investigation and an open report.
