PASADIT KIZAZI HODARI PROJECT

PASADIT as a Sub-grantee to Evangelical Lutheran Church in Tanzania (ELCT) Implements Kizazi Hodari North-Eastern zone Project in five (5) Councils of Tanga region namely; 1) Tanga City Council, 2) Muheza District Council, 3) Korogwe Town Council, 4) Korogwe District Council and 5) Lushoto District Council.

Kizazi Hodari North-eastern Zone activity is a five-year Project as from April 1, 2022 to March 31st 2027 funded by the United States Agency for International Development (USAID).

Kizazi Hodari is a child-focused and family-centred program with the main goal of improving health, well-being and protection of Orphans and Vulnerable Children (OVC) and youth aged 0-17years in high HIV burden communities. The activity contributes to attain the United Nations Program on HIV/AIDS (UNAIDS) 95-95-95 goals through OVC Comprehensive and Preventive services.

In order to maintain high quality level comprehensive services and to meet our beneficiaries and donor’s expectations, we wish to invite applications from suitably qualified candidates to fill the following position:

POSITION: HEALTH AND HIV OFFICER (1 Posts)

Job Title:
Health and HIV Officer (1 post)

Reporting to: Project Manager

Collaborate closely with: RHMT, CHMTs, Health Facility In-charges, CTC in-charges, Clinical Partner i.e. THPS team and CTC data Officers.

Line Manage: Community Case Workers/Community Health Workers.

Locations: Tanga CC, Muheza DC, Korogwe TC, Korogwe DC and Lushoto DC.

Position Summary:


The overall objective of this position is to contribute towards strategic service delivery to OVCs and their caregivers, address critical barriers to service access, uptake, and adherence to scale up impact service delivery, advance progress towards 95-95-95 goals and improve health and social outcomes among OVC and their families.

The job holder will ensure sound and vibrant bi-directional referral and linkage system, improved case identification through various platforms including Faith Community Engagement, linkages to ART and Viral Load suppression among Children and Adolescents Living with HIV/AIDS.

Roles & Responsibilities:

Represent PASADIT and coordinate with the Council Health Management Teams regarding HIV and Health related activities at Councils level.

Visit and hold monthly coordination meetings with Care & Treatment Centers (CTC) in implementation area to support shared confidentiality, facilitate case conferencing, improve the bi-directional referral system, and ensure HIV positive beneficiaries receive needed services at respective Councils.

Support respective health facilities to have updated enrolment registers that demonstrate the cascade of enrolment of CLHIV at the CTCs

Support Community Case Workers (CCWs) to work with high paediatric volume CTCs to trace HIV positive OVC who miss appointments, enrol them into KIZAZI HODARI Project, and link them back to care.

Support CCWs to monitor the clinical status of C/ALHIV and HEI enrolled in the project and provide support needed to improve clinical outcomes, Adherence counselling including; Treatment literacy age-appropriate disclosure support, escorted referral, Palliative care, Mental health and PSS services

Support CCWs to conduct HIV risk assessments for OVC in case management; refer and link at risk OVC to HIV Testing and Counselling (HTC).

Support CCWs to strengthen the capacity of caregivers to support HIV positive OVC/PLHIV and HEI mothers and ensure CCWs refer HIV positive OVC and caregivers to locally available support.

Support the established bi-directional referral system to monitor beneficiaries through the HIV continuum of care as well as service completion for other health and social

Monitor implementation, bottlenecks, performance metrics of the bi-directional referral system; pro-actively offer solutions, conduct quality improvement activities, and implement adjustments to improve referral

Participate in quarterly Council level quality improvement meetings with relevant bi-directional referral system stakeholders; ensure actions from the meetings are implemented and the bi-directional referral system is improved.

Work with the M&E officers to ensure all bi-directional referral data are accurate and produced weekly; use data for decision-making.

Lead the development of a service directory for health, nutrition, and HIV services in implementation area; update the directory at least once a year at Councils Team

Support CCWs to conduct nutrition activities, including nutrition assessments, counselling, and linkage to other nutrition service providers at Councils Team

Provide continuous supportive supervision to CCWs to ensure provision of health-related services and referrals and linkages to

Submit timely updates to the Program Manager for inclusion in the quarterly, semi-annual, and annual reports at Councils Team

Ensure documented VL test results and evidence based decision making on HVL (and document individual CLHIV improvement plan) as well as document lessons learned and best practices for experience sharing and replication.

Perform any other relevant duties as assigned by the Project Manager.

Minimum required Qualifications, Experience and Skills Education:

MUST be:


Diploma holder in Clinical Medicine

Bachelor degree in Health Systems Management, Public Health and Nursing will also be considered.

Male or female not above 35 years of age.

Skills:

Good understanding of the general scope of Tanzania Health specifically HIV related issues.

Passionate to work with OVC and Community Volunteers to achieve expected project results.

Excellent written and oral communication skills in Swahili and English including excellent training skills and ability to work with adults.

Able and willing to work flexible hours as needed without constant or close supervision.

Demonstrate team building, team playing and multi tasking skills.

Demonstrate ability to collaborate and coordinate with a range of stakeholders.

Experience:

At least two (2) years’ experience in a field position with an organization in public health or OVC experience in implementation of community programs in health/HIV, MNCH, TB/Malaria etc., is preferred.

Experienced with PEPFAR funded Projects in Tanzania.

Experienced in USAID donor regulations, systems, and deliverables
