A320 MSN 630 AIRCRAFT INSPECTION AT AEROMAN – SAN SALVADOR AIRPORT

1. The Lessor (Wallis Trading Inc) contracted S.G.I. Aviation Company to inspect the aircraft at Aeroman. S.G.I. Aviation in turn, contracted Aerotech Consulting & Services LLC (AAMTS), a company based in Miami, Florida, to inspect the aircraft and records on its behalf. From discussions with the Inspectors, when they came to see the aircraft, the check, including painting was completed. They only noted some external defects which were still on the aircraft despite the check being completed. They therefore concentrated on inspecting the aircraft records only. They correctly made it very clear to us that their line of communication is as per their Agreement, that is, they will direct any query to S.G.I. Deutch Bank (DB) who we understand now own the aircraft, have one inspector at sight.

2. When the ATCL team arrived at Aeroman, the aircraft was being prepared for a flight test. When the team was authorized to inspect the aircraft, the inspection was confined to the visual appearance of the aircraft, both internally and externally as all access panels were already closed. Several defects were observed. These were documented and passed on to AAMTS with copies to ATCL Management. AAMTS told us that they can only pass these defects to S.G.I. Aviation. Since then, they have neither being given any instructions, nor heard any thing towards actions to be carried out. Among serious defects recorded by ATCL team include the generally poor condition of the cabin interior, defective passenger seats, dirty passenger seat covers, defects in galleys, several fairings cracked in toilets, passenger bulkheads, defects in overhead stowage bins, illegible decals, delaminated thrust reversers 'C' Duct Heat Shield (both engines). None of these defects, and the other defects listed in our report, have been entered into the aircraft records, and hence requiring rectifications.

3. Mr Hadi Akoum has constantly been talking of replacement of passenger seat covers and carpets. This brings the impression that these are the only serious discrepancies. These two items are not sufficient to bring the aircraft into acceptable standards, as per the Lease Agreement. The defects recorded by ATCL have to be addressed fully. In particular, the aircraft interior requires a lot of work, including replacement of ALL passenger seats. The thrust reverser delaminations have to be inspected, recorded and rectifications be carried out as per the Aircraft Maintenance Manual.

4. The Lessor has to acknowledge the defects raised by ATCL and clearly demonstrate how they are going to be rectified.

FINDINGS ACTION REQUIRED
1.0 EXTERIOR APPEARANCE:
The aircraft is painted in ''CREAM''. ATCL's scheme is ''WHITE'' Repainting required in ATCL scheme, i.e ‘WHITE' then apply the logo. There will be substantial down time. Aeroman has started to hint at problems with slot availability. They will give us the estimates for re-painting, applying the logo, interior bilingual markings and any other generated tasks, like a/c weighing
• The wings and horizontal stabilizers have different colour shades and the wings have not been re-painted Wings and Horizontal stabilizer should have one common colour shade

1.1 Rear Cargo Hold:
• The loading is ‘bulk cargo'. Not ‘containerized' Airbus will be contacted on the possibilities of a modification if ATCL elects to have ‘containerized cargo rather than bulk cargo loading. It is normally neat, more secure and faster to deal with containerized cargo. ATCL will have to decide if it would prefer the ‘bulk' or ‘containerized'
• Both door actuator mounting bolts on the door seem to be rotating Inspection to be carried out and maintenance manual procedures followed
• Both actuators exterior housing shows signs of corrosion Maintenance Manual procedures to be followed to clear this problem
• The actuators mounting area dirty Maintenance Manual procedures to be followed to clear this problem
• Vertical stainless plates (fwd and aft) of aft cargo door frames not fitting correctly and one is already dented. Fairings around the plates torn Maintenance Manual procedures to be followed to clear this problem
• The cargo hold interior lower honeycomb panels (both LH and RH) have numerous dents, punctures, dent fillers These have to be replaced because they were abused by the previous operator
• Cargo door fairing fasteners washers missing Maintenance Manual procedures to be followed to clear this problem

1.2 Forward Cargo Hold:
• Both door actuators S/Steel housings corroded Maintenance Manual procedures to be followed to clear this problem
• The cargo hold interior lower steel sheets (both LH and RH) have numerous dents, punctures, dent fillers These have to be replaced because they were abused by the previous operator
• Front cargo door seal lower RH corner worn Cargo door seal has to be replaced

2.0 Fuselage - External:
• LH Wing jack adaptor point - bushes missing Maintenance Manual procedures to be followed to clear this problem
• RH Wing jack adapter point – screw missing Inspect if threads are OK and replace screw

3.0 Aircraft Interior:
3.1 Aft:
• Toilet interior wall lining carries several patch repairs The aircraft is to be delivered to Lessee as per Schedule 1 Part 1 Item 3; i.e. ‘The Aircraft shall be clean by international airline standards'. The interior of the aircraft is dilapidated. There has to be extensive interior refurbishment to bring the interior into this delivery requirement.

In particular, all passenger seats have to be replaced with serviceable ones

The floor carpet has to be replaced with a new one

Cracked fairings, torn seals, damaged wall linings all have to be replaced

• LH Toilet door lock plate damaged
• Vacant/Occupied sign damaged
• ‘LAVATORY' marking not legible
• Aft Attendant's lower stowage door broken
• Rear Galley ovens: missing inserts
• Door knobs broken
• Galley markings/stickers – not legible and some missing
• Trolley locks loose/weak springs
• Lower stowage door locks missing
• Overhead stowage bins seals missing/deteriorated
• Overhead stowage bins: repairs required on the inside of the bins
• Rear galley work table sealant torn/perished
• Rows 16LH and Rows 8/9 LH hat rack rubber fairing worn/torn

3.2 Forward:
• Coat cabinet sliding doors damaged
• Trolley stowage cabinet sliding doors damaged
• Front Galley ovens door knobs broken
• Ovens inserts missing
• One oven unit in G1 galley different from the other 4
• Trolley stowage are, lower fairing (floor level) worn
• FWD galley inboard wall bottom seal retainer has missing fasteners and water sealing rubber strip torn
• FWD galley oven separator adrift at bottom and held by lock wire at top
• Bulkheads carry previous operators markings
• Passenger seats: numerous defects including:
o Seat reclining not working properly
o Loose/excessive play
o Seat arms rests covers worn, delaminated, lifting off
o Seat covers worn and dirty
• Cockpit seats cushions worn and releasing dust, fabric worn
• Sidewall kick carpets old, untidy

Cockpit:
• F/O oxygen mask compartment cover forward end torn
• Fireproof glove compartment cover bumper below door latch missing
• Parking brake lever marking faded/illegible
• Many screws/fasteners missing
• Glare shield trim strips adrift
• Rudder pedal rubber linings torn
• Observer's seat back seat top rear plastic cover torn
• Flood light panel captain's side centre pedestal main PNL & PED knob loose
• Cockpit generally dirty
• Floor mat F/O side, at Oxygen mask compartment/floor junction position badly trimmed, exposing cockpit floor
• Front Toilet several defects including:
o Napkin holders fairing cover lower corner cracked
o Napkin holder stay catches broken
o Toilet door inside covering damaged
o Toilet door side plate near door lock bulging out
o Toilet seat cover damaged
• Floor carpet washed, has patches of discoloration
• Cockpit Gages: Imperial Units (lbs) Aircraft units are in imperial units. ATCL requirement is (lts/kg)

4.0 Engines (LH and RH)
4.1 Both engines (LH and RH) Thrust Reversers inner sleeves are delaminated Approved repairs have to be carried out


CRITICAL ISSUES:

1. Aircraft Paint:
• ATCL will have to incur substantial costs and downtime to repaint the aircraft in ‘white' and install the logo. None completion of all tasks associated with painting may have an impact on the TCAA certification

2. Aircraft Internal Appearance:
An interior refurbishment is required in this aircraft as hereunder:
• The seat structures are in poor condition with: loose seat backs, worn reclining mechanisms, worn armrests
• The seat cushions and covers are worn and dirty
• Class dividers and bulkheads must have existing markings and coverings removed, worn out safety leaflet holders replaced
• Overhead stowage bins, Passenger Service Units, overhead panels need several repairs including application of uniform appearance
• Many seat row numbering decals not legible

3. Engine Thrust Reversers:
• Both engines thrust reversers (LH and RH engines) delamination has to be repaired (permanent repair)

4. Recommendations:
• ATCL should enter into an agreement with Aeroman for the exterior painting and logo application as soon as they give us the quotation and slot time for this work
• It is highly recommended that together with the rectification of all the other cabin defects/refurbishment, passenger seat replacement with new ones be demanded
• Commercial Department should determine the cargo loading preferences (bulk loading vs containerized) and Airbus to give assistance on the possibilities, logistics and costs for such a change (request sent to Airbus)
• Airbus to be requested to lead ATCL in the changeover from imperial to metric units in the aircraft, aircraft operating systems and documentations (message sent to Airbus)
• ATCL Management to promptly establish the cabin interior configuration (LOPA) and have the same approved (probably 16J?/__Y?). The current configuration is 12F/138Y.

Ndege, na sasa hivi baada ya kuikodisha sisi katika mkataba ule wa Dola Milioni 30, ndege ikabidi ifanyiwe matengenzo na kupakwa Rangi za Taifa letu na kuwekwa nembo yetu. Walichogundua ni uharibifu mkubwa ndani ya ndege hiyo yaani sijui walikuwa wanasafirishia nini, lakini viti, milango, n.k vilikuwa ni kuu kuu kuliko umri wa ndege hiyo.

Jaribio la kutaka Wallis Trading (kampuni ya Liberia ambako ndiyo yenye umiliki wa ndege hiyo) kuitengeneza lilishindikana na matokeo yake nini ikabidi tufanyie matengenezo katika "gereji ya ndege" ya Aeroman hapo Al-Salvador kwa mkataba mwingine wa utengenezaji.

Rais Jakaya Kikwete, amewataka wafanyakazi nchini, kukataa kutumika katika miradi ya mafisadi na vigogo wa rushwa ili kuwafanya vigogo hao washindwe kutimiza malengo yao.

Akizungumza na waandishi wa habari ofisini kwake Ikulu jijini Dar es Salaam, Mkurugenzi wa Mawasiliano wa Ikulu, Bw. Salva Rweyemamu alisema badala yake Rais amewataka wafanyakazi kuwa mstari wa mbele katika kuwafichua vigogo hao bila woga.

Alisema iwapo wafanyakazi watagoma kutumikishwa katika `madili` ya kifisadi na rushwa, itakuwa vigumu kwa vinara wa mambo hayo kufanikisha matendo hayo maovu.

Katika kuwapa moyo wafanyakazi hasa wakati huu wanapojiandaa kusherehekea SikuKuu yao ya Mei Mosi, Bw. Rweyemamu alisema Rais Kikwete amefurahishwa mno na kauli mbiu ya mwaka huu inayosema ``Maisha bora kwa kila Mtanzania yanawezekana Mafisadi wakitokomezwa``.

Kutokana na hilo, amesema Rais amewataka wafanyakazi wawe mstari wa mbele katika mapambano ya kutokomeza rushwa na ufisadi.

``Rais anawaomba wafanyakazi, viongozi na wananchi kwa ujumla washirikiane na Serikali katika mapambano haya,``alisema.

Alionya kuwa wafanyakazi hawana budi kutambua kwamba wakikubali kutumiwa na vigogo katika `madili` yao wataonekana watu rahisi na hivyo kudharauliwa.

Alisema Mafanikio juu ya mapambano hayo yanategemea sana ushiriki wa wafanyakazi.

``Rais anawaomba wafanyakazi wasaidie kwa kutoa taarifa za vitendo vyovyote vya rushwa ili zifanyiwe kazi na wahusika wabanwe,``alisema.

Hata hivyo, alisema kwa vile Rais hatakuwepo kwenye sherehe hizo, atakutana na viongozi wa wafanyakazi akirejea safari.

Alipoulizwa ni kwanini mafisadi wanaojiuluzu hawachukuliwi hatua, Mkurugenzi huyo alisema si kazi ya Rais kuwachukulia hatua watuhumiwa hao, bali kuna vyombo husika.

Alisema mtu akishajiuzulu baada ya kutuhumiwa, kuna vyombo vinafanya kazi ya uchunguzi kwanza.
Bw. Rweyemamu alikumbusha kwamba Tanzania inaongozwa na utawala bora unaojali sheria.

Mmoja wa waandishi alimtaka Bw. Rweyemamu atoe ufafanuzi kuwa mawaziri wanaojiuzulu wanafanya hivyo kwa ridhaa yao au wanashinikizwa na Ikulu, hasa ikizingatiwa kwamba wanapotuhumiwa huapa kwamba hawataachia ngazi, lakini baadaye hutangaza kujiuzulu.

Akifafanua, Bw. Rweyemamu alisema mawaziri hao hujiuzulu kwa ridhaa yao. Alisema japokuwa awali huapa kwamba hawatajiuzulu, lakini hufanya hivyo baada ya muda mfupi kwa vile binadamu hugeuza mawazo.

Alikiri kwamba `madongo` ya vyombo vya habari yamechangia sana watu walioapa kutojiuzulu, kubadilisha mawazo.

Wakati huo huo, Rais Kikwete anaondoka nchini kesho kuelekea Addis Ababa kutimiza majukumu yake ya Mwenyekiti wa Umoja wa Afrika AU.

Mbali na kuwa mgeni rasmi katika makabidhiano ya madaraka kati ya Mwenyekiti wa kamisheni ya Afrika anayemaliza muda wake Bw. Alpha Omar Konare, anayemwachia Bw. Jean Ping, Rais Kikwete pia atakuwa na mazungumzo na wafanyakazi wa Makao Makuu ya AU.

Pia katika ziara hiyo Rais anatarajiwa kuongoza kikao cha Baraza la Usalama na Amani la AU na kupewa taarifa juu ya shughuli za baraza hilo.

Bw. Rweyemamu alisema tarehe 30 Rais atakuwa nchini Uganda kwa mwaliko wa Rais Yoweri Museven, kwa ajili ya kutafuta ufumbuzi wa matatizo yaliyoanza kujitokeza nchini Burundi.

Hee mimi nadhani kuna culture ya ufisadi na udanganyifu ulioota mizizi hapa. It looks like everybody in the government is corrupt. Tukianza kuangalia makatibu wa kuu na wakurugenzi mbalimbali hapa nchini tutatapika na kufa kwa kukosa nguvu ya kuhimili kutapika.
