I hereby ask for a help from people who have done it,I am trying to add a contact form for my web project but I failed.Instead of displaying the form,the code for the form were displayed and sometimes it gives me an error message file not found.I would like to use the form with the Captcha mechanism here is the guide I used with no success.

Read between line on the red highlighted words,I was a little confused,the file downloaded has" contactform.php" in the code the reference is done on " contactus.php".Please I will appreciate if I get help from people who know exactly how to do it.
