But invisible to the naked eye, floating in that water, are tiny crustaceans called copepods — also known as water fleas. And inside some of those water fleas are the larvae of Dracunculus medinensis. The Guinea Worm.
She has just been infected.
And she does not know it.
INSIDE THE BODY — THE FIRST TWO WEEKS
Within minutes of being swallowed, those copepods reach the stomach. The strong acids in the digestive system kill them instantly.
But that releases something.
The Guinea Worm larvae. Tiny. Microscopic. Alive.
Over the next two weeks, these larvae do not float harmlessly through the body. They move with purpose. They push through the walls of the stomach and intestines, and burrow deep into the surrounding tissue.
No pain. No fever. No sign at all that anything is wrong.
She goes home. She cooks dinner. She puts her children to bed.
And the worm keeps moving.
THE LONG YEAR OF SILENCE
Now begins the part that makes this parasite unlike almost anything else in nature.
For the next ten to twelve months — nearly a full year — the worm grows silently inside the body. The male and female worms find each other in the deep connective tissue, mate, and then the male dies.
The female does not die.
She keeps growing. Slowly. Continuously. Into a long, thin, thread-like creature — milky white in color. According to the World Health Organization, a mature female Guinea Worm can reach up to one hundred and twenty centimeters in length. That is nearly four feet — living just beneath your skin.
And still — no symptoms. The infected person works, walks, travels, and carries on with daily life.
But inside, the clock is running.
And when the year is up — the worm makes her move. What happens next is unlike anything you would expect from a living creature.
THE WARNING SIGNS BEGIN
Around the one-year mark, the silence ends.
The female worm has finished developing. She is now carrying over three million larvae inside her — and she needs to release them back into water to continue the cycle.
So she begins migrating. Slowly. Toward the surface of the skin — most commonly targeting the lower leg or foot.
The body's immune system, which has been suppressed for months, finally detects her presence.
And it reacts.
The infected person begins to feel a deep, burning pain in the leg. Then swelling. Then fever, nausea, vomiting, diarrhea, and shortness of breath.
This is not discomfort. This is debilitating. People cannot walk. Cannot work. Cannot care for their children. During harvest season — when this disease peaks — entire communities lose their labor force at once.
And it gets worse.
THE BLISTER — AND THE TRAP
A blister forms on the skin. Raised. Painful. Burning hot to the touch.
Inside that blister is the head of the worm, pressing against the skin from the inside.
The burning sensation becomes unbearable. Every instinct tells the infected person to put their leg in water — a river, a pond, a bucket — for relief.
And that is exactly what the worm is engineering.
The moment the blister touches water, the temperature change triggers a biological response. The female worm ruptures through the skin — and releases thousands upon thousands of larvae directly into whatever water source the person is washing in.
The cycle begins again.
The same pond. The same well. The same stream that feeds the village.
Other people will drink from that source tomorrow. Other water fleas will ingest those larvae. And more infections will quietly begin — twelve months before anyone feels a single symptom.

THE ONLY TREATMENT — AND IT IS NOT MEDICINE
Here is the part that shocks most people.
There is no cure for Guinea Worm Disease. No vaccine to prevent it. No drug that kills the worm. And until the blister appears — sometimes a full year after infection — there is no diagnostic test that can even detect it.
So how do you treat it?
You pull the worm out. By hand. Slowly. Over many weeks.
Here is exactly how it works.
The infected area is submerged in clean water. This encourages the worm to push further out through the skin opening. The exposed tip — just a few centimeters — is carefully wrapped around a small stick or piece of gauze. Just enough tension to hold it. Just enough so the worm cannot retreat back inside.
Then you wait.
Day one. The worm does not move.
Day five. A few more centimeters emerge.
Day twelve. Still winding. Still waiting.
The next day, you submerge the wound again. A little more of the worm comes out. You wind it around the stick a little more. And you wait again.
This process repeats every single day for anywhere from ten days to several weeks — depending on the full length of the worm inside the body.
And it must be done with extreme care. Because if the worm breaks — if it snaps during extraction — it dies inside the body and calcifies in the tissue. Joints lock. Deformity sets in. The open wound becomes a gateway for bacterial infection, leading to sepsis or septic arthritis. Conditions that can be fatal.
The worm itself rarely kills. But the complications it leaves behind absolutely can.
PREVENTION IS THE ONLY WEAPON
Because no medicine exists, prevention is not just important — it is everything.
The chain of infection can be broken at several points, and every single one requires community knowledge.
First — filter your drinking water. Even a simple cloth filter, or a pipe filter made from nylon mesh, is enough to remove the copepods from water before it is consumed. No copepods. No larvae. No infection. A piece of cloth — that is all it takes.
Second — never enter a water source when you have an active blister or an emerging worm. This single act is how Guinea Worm travels from one body back into the community water supply. Containing this moment breaks the entire cycle.
Third — treat stagnant water sources with a safe larvicide called temephos. It eliminates infected copepods without harming the humans who drink from the same water.
Fourth — and most powerful — education. Communities that understand the one-year delay between infection and symptoms are far better equipped to protect themselves. Because by the time the pain appears, the source of infection was a decision made a full year ago. You cannot feel it coming. You can only prevent it from starting.

The Carter Center confirmed that in 2024, only thirteen human cases were reported worldwide.

We are standing at the edge of making Guinea Worm Disease only the second human disease ever eradicated from the planet — after smallpox. Eradicated not with a vaccine. Not with a drug. But with knowledge, clean water, and community action.
The fight is not over. Animal reservoirs have complicated the final push. Six countries in sub-Saharan Africa remain endemic. And this disease requires no medicine to spread — only contaminated water and a community that does not know what to watch for.
