thank you for sharing such a wonderful and helpful information, surely, if most of us follow our instinct in decisions making we will be somewhere but if we follow some of defined ways of making decision we will be in a place/position that we wanted to be.
i second you when you said most of decision makers do not follow these defined ways of making decision, and these are the people who get chances and opportunity to attend workshops, seminar and what ever that might be that they get to make a good decision of which they don't. most of these decision makers decide based on "what is in it for me" instead of organization or company or country (at large) betterment's. it is a shame for these people who are paid to think.
the blame has not to be sent to decision makers alone, it has to be sent to the whole system that is used to give such people the room to be in such posts, most of these people are there based to the relationship they have with the selection team or any big potato, again it is a shame, we need to decide together on better ways that will improve each others life and not individuals.
a good decision making is a good engine for any family, organization, company or country, i challenge you people including myself to learn better ways to make not only a good decision but also the right decision.
