Chelsea have confirmed that John Terry and his agent met with chairman Bruce Buck and director Marina Granovskaia this week and an offer was forthcoming.
"With it coming so late in the season, this is a big decision for John and his family and it is something that they are now considering," said the club.
John Terry announced in January that he had not been offered a new contract and expected to leave Chelsea.
The 35-year-old defender, who joined the Blues at the age of 14 and played over 700 games for the club, has won four Premier League titles, five FA Cups and the Champions League with the Blues.
"Ideally, I would have loved to stay, but the club's moving in a different direction," he said at the time.
