    THE government is working on a strategy that will ensure Senior Citizens in the country receive medical treatment free of charge.

    The Permanent Secretary in the Ministry of Health and Social Welfare, Ms Blandina Nyoni, told the ‘Daily News’ in Dar es Salaam on Monday that the strategy will not allow for loopholes for people to take advantage of the opportunity for the elderly.

    “The elderly are supposed to receive medical treatment free of charge, however, we did not have a clear system that identifies them, and that is part of the government’s strategy to improve health services in the country,” she noted.

    Although the elderly are meant to receive free medical services, the opportunity was abused by some who were able to pay for the health service.

    And some health centres were turning senior citizen away preferring to serve those paying cash.

    With the new strategy which will be managed by the National Health Insurance Fund, it would be difficult to abuse the window provided for the senior citizen and those deserving to receive free medical services will be easily identified.

    Mkuu the problem is how do we defined the "elderly" in Tanzania? According to the BBC via UN statistic the life expectancy in Tanzania is 57 (male) and 59 (female). So if you look at it most Tanzanians do not reach what is considered an elderly age to begin with. So who are we going to classify as the elderly?

    The reason I ask this is because if you take an example of the United States. A person starts receiving Medicare (there health plan for the elderly) at the age of 65 which also happens to be the retirement age in America. Now in Tanzania our life expectancy average for both male and female is 58 which is below our retirement age of 60. Hence most Tanzanians die before they reach retirement age.

    So if you look at it mkuu it is the population which is not the "elderly" which needs more medical care than those who are actually lucky enough to reach an age where they can be considered as elderly. Yes morally and socially it is right to give free medical treatment to the elderly but statistically speaking what does it do for the overall health of the country? Are we going to say to the people that you are on your own if you are lucky enough to become an elder then we will help you.

    If anything mkuu I think all children under the age of 18 should be a health priority and specifically those under the age of five because that is where our mortality rate is high. Let's try and create a health care system where most of us will make it to that so called elderly age. Otherwise are we going to chose people who are luckier than most of the population to begin with or the small children and young mothers who die during labor? It is a very complicated issue.

    What the government should do is overhaul the entire healthcare system to benefit a majority of the population. If we only look at the elder who only a few of us will be lucky enough to grow and be classified that way then we will have a nation where strong able bodies people die even before they reach retirement but a very small segment of the population will make it well into their eighties and maybe nineties because they were lucky enough to grow and be classified as elders.     Mwanafalsafa umekuja na point nzuri sana suala la umri; kama life expectancy ni chini ya 60 mzee basi atakuwa anamiaka kuanzia 45-50??

    And that is exactly my point mkuu. Mzee tuta mdefine vipi? Halafu the funny thing about as Tanzanians is that we call a person who is 45-50 "kijana" when that "kijana" will likely have about 10-15 more years to live. How is that possible?

    In countries where life expectancy is between 75-80 a person is middle aged when they are about 40. A middle aged person is no longer a youth. For us according to our life expectancy we reach middle age when we are about 30 (kumbuka middle aged person hatakiwi kuitwa kijana tena) but a person still remains a "youth" until they are 40???

    If we want this health care for the elderly to reflect our life expectancy then as you say mkuu mzee will have to be from about 45-50 or at least 50-55. Which will also be kind of unrealistic because you are going to be giving free medicare to people who are actually still part of the work force??? Isn't the whole point of this policy to help our population which is no longer working?

    Otherwise instead of free healthcare for the elderly let's change the policy and make it free healthcare for the poor and give free health care to people below a certain income. Because it wouldn't make sense giving free healthcare to people above 60 because many of us sadly won't make it there and it won't make sense giving free health care to people still in the working force and can afford it.

    These are all things which need to be considered but as you know we Tanzanians just copy and paste everything without even thinking about how it implies to our own situation reminding me of our "terrorism policy" which we copied from the US.

    Our retirement age does not reflect our life expectancy. Let me use an example of the U.S.A. again. In the U.S. retirement age is 65 and their life expectancy as per 2010 is 78.4. Meaning a person is expected to live at least a good 10 years after they retire. Sasa sisi a person is expected to die even before they retire. Our life expectancy is too low.

    Ndiyo maana mimi nasema instead of only targeting "wazee" let's target the poor people instead who can't afford healthcare. Our life expectancy is too low for us to categorize some of our policies. Does it make sense kweli life expectancy kuwa lower than the retirement age? But then again you can't make people retire at 50 because we are too poor that huyo mtu after 50 ata jihudumiaje? it is very complicated.

    Mtazamaji tufute kabisa kusema huduma ya bure wapewe wazee. Kwa sababu life expectancy yetu ipo low tutarget watu masikini awe mtoto, kijana au mzee. Let's target the poor so we increase our life expectancy to begin with because it wouldn't make sense targeting only people who have surpassed the life expectancy age already.

    Mwanakijiji swali lako ulilomaliza nalo ni zuri lakini sikubaliani na kichwa cha habari kwani kama alivyosema Katibu Mkuu wa Wizara sera ya kutoa msamaha kwa baadhi ya makundi ya wagonjwa (watoto, wazee, wafungwa, maskini), aina ya magonjwa (mf. magojwa sugu) na aina ya huduma (mfano za ukunga na uzazi) ilikuwepo hivyo si ya CHADEMA. Aidha mlengo wa CHADEMA ulikuwa si huu wa ku- target makundi yanayostahili kupata msamaha, wao katika Afya na Elimu waliongelea 'bure' kwa watu wote.

    Narudi kwenye swali:

    Jambo la kwanza unapokubali kutoa msamaha katika huduma yoyoye ya jamii lazima uhakikishe kuwa kuna mfumo mzuri wa kuwatambua au kuwatambulisha hao walengwa bila kuleta utata au ulahisi wa kudanganya. Kuwatambua watoto, Wafungwa, wanaokuja kujifungua na wenye magojwa sugu hili linaweza kuwa rahisi kuwatambuwa maskini hili lilikuwa 'vague' sana. Unajua si kila mstaafu ni maskini.

    Mtu anaweza kustaafu akiwa na pension yake nzuri na akaendelea na kazi za kujiajiri za ushauri kama Consultant na akawa aliwekeza katika miradi kadha wa kadha na pengine ukakuta tena alisomesha vijana wake vizuri na wanamwongezea kipato. Mtu kama huyu unaweza kukuta pale kijijini ndiye tajiri sasa ukisema eti unampa msamaha kama maskini kuliko wengine ni kichekesho kwa raia wengine ambao kweli ni maskini pale kijijini na kuendeleza tabaka la wenye nacho!

    Jambo la pili ambalo ndilo limekuwa changamoto kubwa ni kwamba tunajua kuwa hakuna huduma ya bure popote duniani hivyo ukiweka msamaha lazima uweke hiyo provision au tuseme utafute hilo fungu litakalotumika kuwasaidia hayo makundi vinginevyo inakuwa kuweka misamaha kisiasa tu kitu ambacho kinashindikana kutekelezeka huko Wizarani na kwenye mahospitali yenyewe. Ushari ni kufanya tafiti na makisio mazuri ya kisayansi ya idadi na gharama za huduma kwa unaotaka kuwasamehe kutokana na uzee (binafsi ningebaki kwenye miaka 65) na kutenga hilo fungu husika. Ni vyema hili lifanyike katika ngazi ya kila Halmashauri na baada ya matibabu, bima ya afya ilipe gharama za matibabu hayo kwa vituo vya afya vilivyowahudia wazee hao ambao watakuwa wamepewa vitambulisho maalumu.

    Ukweli wazee wana umasikini wa kipato kikubwa sana kuliko age group nyingine . Kugeralise umaskini tu sidhani kama itakuwa sahihi. Anyway Who is Mzee in tanzania standards.Where are most wazee living?

    Serikali inabidi iwaaangalie maana huwa najiuliza mbona na sisikiii NGO za wazee kama zilivyo za watoto , kina mama etc.

    First of all, I think serikali hapa inataka kukurupuka kufanya jambo ambalo haina uwezo wa kuli-implement vizuri. And I don't think huyu Ms Blandina Nyoni knows what she is talking about? I mean, waulize nchi zilizoendelea kama vile Uingereza na Marekani watakueleza ilivyo ghari kutoa huduma za bure kwa wazee. You know magonjwa makubwa yanayowasumbua wazee kama vile Diabetes, Parkinson, Cancer, Damentia, na magonjwa mabalimbali ya moyo mostly yanahitaji sustainable long-term care and advanced modern medicines which is very, very expensive kwa nchi masikini kama Tanzania kumudu!

    Jeez, bado kwanza hatujatatua/solve basic problems kama vile uhaba mkubwa wa wauguzi, madawa, na vitendea kazi vya kutosha katika mahopitali na kliniki zetu,lakini tunataka tu ku-adopt mfumo wa nchi zlizoendelea ili nasi tusifiwe kuwa tumeendelea bila kwanza kukaa na kutafakari kwa undani jinsi tutakavyofanikisha hilo zoezi.

    Binafsi, ningeishauri serikali, instead, itumie gharama na muda mwingi kuhudumia waototo na akina mama wajawazito kwa kutibu magonjwa/matatizo yanayoweza kuzuilika kwa njia rahisi huku ikiwekeza katika kuielimisha jamii kuhusu usafi binafsi na kanuni za msingi zinazoweza kuzuia magonjwa (kunawa mikono, kuchemsha maji ya kunywa, na kufanya mazoezi). Tukumbuke: kinga ni bora kuliko kutibu.

    I mean, sina kinyongo na wazee, lakini at least wao tayari walishapata muda wa kuishi na kufaidi mambo ya dunia hii. Vipi kuhusu watoto na akina mama wajawazito ambao wanakufa in a large number kwa kukosa huduma za msingi za kiafya? I think hao ndo' ambao serikali inapaswa kuwasaidia zaidi (kuwapa kipaumbele zaidi).

    Unajua mkuu kama nilivyo sema hapo juu kwa Tanzania kuja na definition ya mzee it is a complicated matter. Kijamii kwetu mzee ni tuseme labda mtu yoyote mwenye umri wa miaka 60 kwenda juu kwa sababu tumemuona mtu kastaafu, ana mvi na labda wakati huo tayari ana wajukuu. Hapo sisi ndipo tunapo muona mtu ni "mzee".

    Ila realistically sisi Watanzania tuna kufa kabla ya kufikisha hata umri wa kustaafu ambao ni miaka 60. Kwa ukisema wazee tu ndiyo wapewe kipaumbele ina maana una wapa kipaumbele watu wenye bahati zaidi kwenye jamii ambao ndiyo hao waliobahatika kuitwa wazee. Meaning kwa kuprioritize wazee tu hausaidii kuimprove general health ya wananchi wala kuincrease life expectancy. Mimi naangalia kijamii yote zaidi.

    Sasa kama wengi wetu tuna kufa hata kabla ya kufikisha 60 hiyo free healthcare kwa wazee inaanzia umri gani? Je tuhesabu na watu ambao wapo in their late 40s to mid 50s kama wazee? Je wewe ukipewa udefine "uzee" kwa Tanzania utasema uanzie umri gani. Na ni Watanzania wangapi wanao fikisha huo umri? Kwa sababu mimi bado haiingii akilini retirement age iwe 60 wakati life expectancy yenyewe siyo 60.

    Mimi naomba niwe bepari kidogo kwenye hili. Nina maana kuwe na system ya kuchunguza wazee hao walikuwa wanaishije kwenye jamii. Maana kuna watu wao ni kujirusha kwenda kuchwa na hawajishughulishi na shughuli yeyote. Hawa nao kuna siku watakuwa wazee. Je tunahitaji kodi zetu ziwatibu watu wa namna hii?

    Mimi kwa mtazamo wangu nazani tungeanza na kwa watoto wa umri wa miaka 0 -5/10 kwani ukweli ni kwamba watoto ndio walio wengi na kinachotekea hawa ndio wanafariki sana sasa wakija kupiga mahesabu ya AVERAGE life kwa MTANZANIA nakuwa inashuka hadi hapo 55 kwa sababu ya VIFO vya hawa watoto.

    So tatizo ni vifo vya watoto na naamini watanzania wengi wanvuka huo Umri wa 55 ila Tatizo ni kuwa Calcylation zake ni Average ya population yote na inakuwa ina athirika na VIFO VINGI VYA WATOTO.

    Uko sawa lakini tukitaka kupata defintion sahihi ya mzeee tunayetaka kumsadiai tunatakiwa kuangalia beyong age?

    Nimelisoma wazo lakini ingawa kuna complication katika kuwatambua lakini bado ni wazo sahihi . Hii complication ni kama ya wanafuzi wa vyuo vikuu kupata mikopo. nadhani tujairibu kupendekeza njia sahihi za kuweza kuwaweka watu katika kundi hili la "wazee" wanahitaji msaada.


    • Mfano mzee mwenye miaka 50 yuko dar na mwingine ana miaka 50 yuko mAnyoni. Kama ni definiton sahii basi huyu mwenye makazi manyoni ni ndio anaqaulify kupata upendeleo wa afya .
    • Kama mzee ana nyumba zake mbili anapata kodi mwingine anategemea wajukuu na watoto. yupi ana qualify
    • Kuna wazee ambao walikuwa wakulima hawakuwai kupata mishahara na ajira za ofisini na wameishiwa nguvu
    Kwa hiyo tukichanganya umri, location, vipato, rasilimali na tukiwashirkiza viongozi wa mitaa tunaweza kupata wazee sahihii wanaotakiwa kusaidiwa.
