Ndugu "harakat", in response to your concern, i would like to give out two definitions (actually according to Yahoo Answers) which can tell us whether it was an accident or just a "mihap". Then, latter on Swahili people will help to give us "a swahili term" which will be used to the incident like this.

1.Accident involves two things. That is collusion of two vehicles, one vehicle and pedestrian etc., and Mishap involves malfunctioning of one mechanism. Mishap is an incident. (For example: a boat falling into a lake, a bus slipping from a ghat section of a mountain are mishaps and not accidents).
2. Accident is an unexpected event. Mishap is avoidable if due diligence is exercised and if fore-casted effectively.
